Financial Qualification

Factor	Criteria
Audit Report	Bidder has to submit financial statements certified by an independent auditor as required in ITB Clause 15.2(a) for the period specified
Other documents proofing its financial standing	Bidder has to submit other documents proofing its financial standing, as required in the BDS and in accordance with ITB Clause 15.2(b);
Average annual turnover	The average annual turnover for the period ———– does not exceed the amount of the financial proposal of the Bid in value specified in the BDS.

Legal Qualification

Factor	Criteria
Conflict of interest	A Bidder shall not have a conflict of interest. All Bidders found to have a conflict of interest shall be disqualified.
Business organization registration certificate or valid trade license	Having been submitted valid trade license or business organization registration certificate issued by the country of establishment in accordance with ITB Clause 4.6.(c)
Delivery and Completion Schedule	Bidder has to complete the Delivery and Completion Schedule through Electronic Government procurement System as per template in Section 6, Statement of Requirements.
Nationality	Bidder have nationality in accordance with ITB Sub-Clause 4.2;
Valid Tax Clearance Certificate	Domestic Bidder has to submit a valid tax clearance certificate issued by the tax authority, in accordance with ITB Clause 4.6(b)(iii);
Form Data on Joint Ventures	The Bidder has to submit the Form Data on Joint Ventures, the Agreement governing the formation of joint venture, or letter of intent to form JV (ITB 36.2(J))

Professional Qualification

Factor	Criteria
Professional Certification of Compliance Form	Bidder has to provide in the Bidder Certification of Compliance Form information related to its professional qualification and capability for the period specified in the BDS Clause 14.1; and ITB 36.3(b)
Professional practice certificates	Bidder has to submit relevant professional practice certificates, Clause 4.6(b)(iv);

Technical Qualification

Factor	Criteria
Descriptive technical literature	Bidder has to submit Descriptive technical literature
Origin of the Goods and Related Services offered	Bidder has to provide the Statement attesting the origin of the Pharmaceutical and Medical Equipment and Related Services offered
Description of the organization of the warranty	Bidder has to submit Description of the organization of the warranty offered in accordance with the conditions laid down in GCC Clause 23
Satisfactory execution of contracts	Bidder has to submit Certificates of satisfactory execution of contracts provided by contracting parties to the contracts successfully completed in the period and budget of at least that of this contract in the past years;
Manufacturer Authorization	Bidder has to submit Manufacturer Authorization Letter in accordance with ITB Clause 5.6
