Bidder has to provide the Statement attesting the origin of the Pharmaceutical and Medical Equipment and Related Services offered

For Isoniazid (INH) – 300mg – Tablet of 24×28, & Isoniazid (INH) – 100mg – Dispersible Tablet of 10×10

For Item Isoniazid (INH) – 300mg – Tablet of 24×28 & Isoniazid (INH) – 100mg – Dispersible Tablet of 10×10

The manufacturer shall be listed in the latest updated version of List of Tuberculosis Pharmaceutical Products classified according to the Global Fund Quality Assurance Policy before or upon bid opening date will be given first priority.

For Fluconazole – 200mg – Tablet of 100 & Amitriptyline – 25mg – Tablet of 100

For Fluconazole – 200mg – Tablet of 100 & Amitriptyline – 25mg – Tablet of 100

For Foreign bidders:-

• Bidder with valid EFDA product registration certificate will be given a first priority.
• Bidder with valid SRA and EFDA GMP certificate will be given second priority. 
• Bidder with valid EFDA GMP certificate/SRA certificate will be given third priority.

For Local bidders

• Bidder with valid manufacturing license and valid EFDA registration certificate /Valid EFDA GMP certificate/ valid EFDA GMP exemption letter/ bidders name listed for the item on the letter sent from EFDA, will be given first priority.
