Form Data on Joint Ventures	In the case of a bid submitted by a joint venture (JV), the Bidder has failed to submit the Form Data on Joint Ventures, the Agreement governing the formation of joint venture, or letter of intent to form JV, including a draft agreement, in accordance with ITB Clause 4.1
Nationality	Nationality in accordance with ITB Clause 4.2
Valid tax clearance certificate (Domestic Bidder)	Having been submitted valid tax clearance certificate issued by the tax authority (Domestic Bidders Only) in accordance with ITB Clause4.6(b)(iii)
VAT registration certificate (Domestic Bidder)	Having been submitted VAT registration certificate issued by the tax authority (in case of contract value of Birr 100,000.00 and above) in accordance with ITB Clause 4.6.(b)(ii)
Valid business license	“Not having been debarred by decision of the Public Procurement Agency from participating in public procurements for breach of its obligation under previous contracts in accordance with ITB Clause 4.6(b)(i)

Terms and Conditions: የአልጋ አጋልግሎቱ የምሰጥበት ቦታ ሮቤና ጎባ ከተማ ዉስጥ መሆኑን አይዘንጉት የአልጋ አጋልግሎቱ የምሰጠዉ እንግዶች ወደ መወዩ ሕ/ናና /ጤ/ሣይ/ኮ/ሪ/ሆስፒታል በሚመጡበት ወቅት ናዉ የአልጋ አገልግሎቱ ለአሸናፍዉ ድርጅት አስቀድሞ በደብዳቤ የምገለፅ ይሆናል . አሸናፍዉ ድርጅት ከመ/ቤቱ ጋር ዉል ይፈፅማል የዋጋ ማስተካያን በተመለከተ ከ ኢት/ስታትክስ አጋልግሎት በሚጋኝ የዋጋ መረጃ እና ከዝያ ከልታጋኛ በአከባቢዉ ያለዉ ወቅታዊ ዋጋ በመ/ቤቱ የገበያ ጥናት ባለ ሙያዎች ተጠንቶ በምቀርበዉ የምፈፀም ይሆናል መ/ቤቱ ጨረታዉን በከፊልም ሙሉ በሙሉ የመሠረዝ መብቱ በህግ የተጠበቀ ይሆናል የዉል ግዜዉ ለ2019 ዓ.ም በጀት ዓመት የሚያገለግል ይሆናል
